Responsibilities/

Positions Summary:

 

 

The Clinical Pharmacist Specialist must be a licensed pharmacist, with a Pharm.D., preferably with a residency or equivalent experience.  Incumbent is responsible for planning, implementing and operating drug monitoring and utilization programs, as assigned.  Incumbent functions as the Clinical Pharmacist expert in all areas related to their respective area(s) of coverage and functions as a consultant to other health care professionals.  Incumbent is responsible for developing, providing, and supporting educational programs, as assigned, for Pharmacy Service.

Medication Dispensing and Distribution:  Inpatient/outpatient orders are checked for pertinent information.  Identifies, interprets, and resolves prescribing and dispensing issues and documents interventions as necessary.  Dispenses controlled substances with accuracy.

Clinical:  Participates in daily multidisciplinary team rounds (as applicable).  Interviews and evaluates patients upon admission and during hospitalization for appropriateness of pharmacologic therapy and makes a pharmaceutical care plan as appropriate.  Develops criteria for utilization of new medications (as assigned) for approval. Designs and/or participates in research studies or outcomes research involving drug therapy or clinical pharmacy functions (as assigned).  Provides care and/or services appropriate to the age of patients being served.  Designs and coordinates drug utilization evaluations including the collection and compilation of data, preparation of reports, and presentation to the appropriate committee (as assigned).

Education:  Serves as a preceptor for final year pharmacy clerkship interns/residents (when applicable).  Participates in the development and implementation of formal/informal drug education/clinical consultation to medical, surgical, nursing, and pharmacy staff.  Assesses educational needs of Pharmacy staff in consultation with Pharmacy supervisors.

Administration/Technical Support:  Responsible for adverse drug reaction (ADR) reporting.  Works with other health care providers to develop programs for improvement of drug use.  Develops and conducts drug usage evaluation in areas of need from a cost and patient outcome perspective.  Documents cost reduction/cost avoidance activities on a periodic basic.  Manages the hospital formulary and ensures its appropriate utilization by health care providers.  At the lower grade level, incumbent will perform the duties described for the full performance level position, but with closer supervision and guidance provided.

 

Qualification Requirements Used:

All individual applying under this announcement must meet the qualifications for appointment as contained in VA Handbook 5005, Part II, Appendix G15 dates April 15, 2002.  This standard is available at your local Human Resources Liaison Office for review.  Candidates who meet the appropriate requirements contained in the standard will be considered.

 

Basic Requirements:

Citizenship:  Must be a United States Citizen (Non-citizens may be appointed when no qualified citizens are available in accordance with current regulations).

 

Education:  Graduate of a degree program in pharmacy from an approved college or university.  The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE), or prior to the established of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP).  Graduates of foreign pharmacy degree programs meet the education requirements if their degree is found to be equivalent to degree programs recognized by the ACPE.  This finding may be based on any of the following:  A letter of acceptance into a U.S. graduate pharmacy program recognized by the ACPE; written certification from the Foreign Pharmacy Graduate Examination Commission, stating the individual has successfully passed the Foreign Pharmacy Graduate Examination; a letter from a U.S. College or university with a pharmacy degree program recognized by ACPE stating that the individual’s foreign pharmacy degree has been evaluated and found to be equivalent to its Bachelor of Pharmacy degree.  Transcript must be provided with application package.

 

Residency:  Completion of a hospital residency recognized by the American Society of Hospital Pharmacists. 

 

License Must have, full, current and unrestricted license to practice pharmacy in a State, Territory, or Commonwealth of the United States (i.e., Puerto Rico) or the District of Columbia.  Proof of licensure must be provided with application package.

 

Physical Requirements Must meet physical requirements as required by VA Directive and Handbook 5019.  Physical examination with positive results required.

 

English Language ProficiencyMust be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).

 

Experience or Education

Completion of one of the following:

 

1.   The equivalent of 1 year of experience at either of the next two lower grade levels; or

2.   Completion of a 6-year Pharm.D. or formal post-baccalaureate (M.S., Pharm.D.) hospital oriented degree program recognized by ACPE.

 

Knowledge, Skills, Abilities required for GS-11 grade level:

In addition to the experience or education required above, pharmacists at this grade level must demonstrate the following knowledges, skills, abilities and other characteristics:

 

1.      Ability to read, interpret, and apply complex written instructions;

2.      Knowledge of professional pharmacy practice;

3.      Ability to communicate orally and in writing;

4.      Basic knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherpeutics;

5.      Basic computer skills;

6.      Ability to analyze drug related medical problems;

7.      Skill in preparing sterile products; and

8.      Knowledge of standards related to distribution and control of scheduled and non-scheduled drugs (including research and investigational drugs.)  This includes, but is not limited to, those established by the Drug Enforcement Administration (DEA), Food and Drug Administration (FDA), Department of Veterans Affairs (VA), the State, (Territory or District of Columbia, if appropriate), and the Joint Commission on Accreditation of Healthcare Organizations (JCAHO).

 

Knowledge, Skills, Abilities required for GS-12 grade level:

Completion of the equivalent of 1 year of creditable experience at the next lower grade level which is directly related to the position to be filled.  In addition to the experience or education required above, pharmacists at this grade level must demonstrate the following knowledges, skills, abilities and other characteristics:

 

1.       In depth knowledge of a specialized area of clinical pharmacy practice or a specialty area of pharmacy such as quality assurance/utilization review, ADP systems, drug information, etc.;

2.       Advanced knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics;

3.       Skill in monitoring and assessing the outcome of drug therapies including physical assessment and interpretation of laboratory and other diagnostic parameters;

4.       Knowledge of the design, conduct, and interpretation of controlled clinical drug trials or other research related to health care;

5.       Ability to communicate orally and in writing with a wide variety of individuals.  This would typically include beneficiaries, professionals in other health care disciplines, and health related groups in the community; and

6.       Knowledge of the standards related to distribution and control of scheduled and non-scheduled drugs (including research and investigational drugs.)  This includes, but is not limited to, the standards established by DEA, FDA, VA, the State (Territory or District of Columbia, if appropriate), and JCAHO.
